  -The worksheet automatically calculates your cost per pack as you enter the quantity of each item you choose. You will see the running total of your cost, suggested selling price and potential profit per pack.
  -Print this Worksheet; you will need it when placing your order.
  Fill out a separate worksheet for each classroom supply list.

  -Fill in the following areas with your supply pack information;

  list of supplies included
  supply pack cost
  dead line for ordering packs
  supply pack pick up date
  who to make checks payable to
  
  -Print as many forms as you need and distribute them to students to take home.
  Fill out a separate parent order form for each classroom supply list offered.
  
  Once you’ve collected all the parent order forms and payments, you’re ready to place your order!
  
  
• ORDER YOUR SUPPLY PACKS

  -Fill in customer contact information, quantity of packs & date packs are needed.
  -Print this form and fax or mail it along with your original worksheet(s).

Minimum order – 10 packs of one configuration.
No deadline -There is no deadline for ordering however, please allow 6 weeks delivery.
Payment is due 30 days after supply packs are shipped to you.
Returns accepted on packs but will be charged a 15% re-stocking fee.
Additional Supply Packs - If you find you need more packs after your order has been shipped, call us and refer to your invoice number. An additional $2.99 will be added to the cost of each additional pack.
